I hope you can make it to the next ACAP meeting. As you all know I took care of parents for over 30 years. We all need help every now and again.

Leaving the hospital can feel overwhelming, especially when you’re suddenly responsible for medications, follow-up appointments, home safety changes, and making sure your loved one doesn’t end up right back in the hospital.

Our July ACAP Dallas-Collin Counties program will help families feel more confident and prepared after a hospital discharge.
